Most-recent electricity bill for the service being applied for
Valid government-issued ID containing the signature and address of the consumer
Certification from the local SWDO issued within six (6) months - (For Non 4Ps Lifeliner/Renewal)

Other Supporting Documents

If electric service is not registered under the name of the applicant: Proof of Residence
If the application is filed through a representative: Letter of Authorization
Valid government-issued ID (with signature) of the representative
In case of transfer of residence: Re-issued Certification from the local SWDO
